What Questions Should I Ask

OK, here's the deal....
I've landed the job of conducting a survey of our league members about playing in the league. I've got questions that I thought up, but I was wondering what questions you would want to be asked on a survey meant to help improve and grow a league.

Any ideas or suggestions would be great.

Here is what I have so far:
